Record workplace accidents using a guided six-step wizard. Capture injuries, treatment, and witnesses. Automatic RIDDOR check determines reporting obligations.

1. Step 1: Injured Person Details

Record the full legal name, job role, and date of birth of the person who was injured. Full Name is required to save the record.

2. Step 2: Incident Details

Record when and where the accident happened, a full description, and any witnesses. Date & Time and Description are required fields.

3. Step 3: Injuries

Use the interactive body mapper to record each injury. Click a body area on the diagram, then describe the injury type and severity. Multiple injuries can be recorded. The injury data feeds directly into the automatic RIDDOR check on Step 5.

4. Step 4: Treatment

Record each treatment given. For each entry capture the treatment type, who provided it, where it was given, and any notes. Multiple treatments can be logged.

5. Step 5: RIDDOR Check

RIDDOR (Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 2013) requires certain injuries to be reported to the HSE. The system automatically determines whether the accident is RIDDOR-reportable based on the injuries you recorded in Step 3. The result is clearly shown as REPORTABLE or Not reportable. If reportable, you can mark the report as submitted to the HSE and record the reference number.

6. Step 6: Review & Save

The review step shows a summary of all entered data. A warning is shown that the record will be permanently saved and cannot be deleted (GDPR compliance). Review all details carefully before clicking Save Accident Record.

Accident records are permanent
Once saved, an accident record cannot be edited or deleted. This is by design for legal compliance. Make sure all details are correct before clicking Save Accident Record on the final step.
RIDDOR deadlines
Fatalities and specified injuries must be reported to the HSE within 3 days. Injuries requiring more than 7 days off work must be reported within 15 days. Over-3-day injuries that do not require reporting must still be recorded. The system identifies which category applies.
You can navigate back between steps
All six step tabs are clickable — you can jump back to an earlier step to add or correct information at any time before saving. The record is only saved when you click Save Accident Record on the Review step.
Common Mistakes
Rushing through the injury step and recording too few details — the RIDDOR check depends on accurate injury data.
Forgetting to mark RIDDOR-reportable accidents as submitted and record the HSE reference number.
Not adding witnesses — witness names are important evidence in any later investigation or insurance claim.
