How to Fill It In

Note the type of waste, how it was produced, and select the correct EWC code.
Look up EWC codesRecord both parties
Producer details (name, address, SIC code) and carrier details (name, registration number).
Sign and store
Both parties sign (electronic signatures valid since 2011). Keep records for 2 years minimum.
